Fee Approval

If you have received a prepayment invoice for records and would like to submit a guarantee of payment, Sharecare Health Data Services will review your request and respond accordingly. By submitting fee approval below you are accepting the terms of the invoice and acknowledge that payment will be released to Sharecare Health Data Services within 30 days of the invoice date.

Please note that fee approvals are accepted for accounts in good standing.

  • Invoice number is 5 digit alphanumeric or a completely numeric set of digits.
